Those two facts sitting next to each other should trouble us more than they do.

Part of the answer is that we are treating the wrong layer. Conventional medicine is extraordinarily good at acute disease: a broken bone, an infection, an appendix that needs to come out. It is built around identifying a single cause and removing it. Chronic disease doesn’t work that way. Type 2 diabetes, cardiovascular disease, what I see in practice; these conditions rarely have one cause. They accumulate, usually over decades, from a combination of what someone eats, how they move, how they sleep, how connected or isolated they are, and how their body has been quietly inflamed by all of it for years before anything shows up on a scan.

Treating the result, the elevated blood pressure, the joint that finally gives out, without addressing what produced it, is a bit like repainting a wall that keeps damping through. It buys time, but it doesn’t fix the wall.

This is why you shouldn’t think of the six pillars as lifestyle advice, but as root-cause therapy. Physical activity, nutrition, restorative sleep, social connection, stress management, and avoiding harmful substances; each one has a direct, mechanistic effect on the chronic inflammation and metabolic dysfunction that sit underneath most of what we now spend our healthcare budgets treating. The evidence linking sleep quality to cardiovascular mortality, or plant-forward eating patterns to reduced stroke risk, is as robust as anything else in modern medicine. It just doesn’t get treated with the same seriousness, possibly because it doesn’t come in a prescribing pad.

What makes lifestyle medicine genuinely medical is that it still requires the same rigour: a proper history, an understanding of mechanism, evidence behind every recommendation, and a willingness to say when something isn’t yet proven. And what it adds is a longer view. Instead of asking what’s wrong right now, it asks what’s been quietly going wrong for the last fifteen years, and what would need to change for that trajectory to turn elsewhere.

If 80% of chronic disease is avoidable, the real question is why healthcare still isn’t built around prevention.

Medicine has never advanced faster, and chronic disease has never been more common.
